High Spinal-Total Spinal

Prevention

  1. Spinal:
    • Use heavy Bupivacaine.
    • Provide the correct dose of 1.8ml heavy Bupivacaine. 
    • Inject at the L3/4 or L4/5 level.
  2. Position:
    •  

How to Recognise

  1. The spinal block is higher than > T6.
  2. The patient may have upper limb weakness.
  3. The patient may a weak arm grip (C8 – T1).
  4. There will be hypotension and bradycardia (T1 – T4). 
  5. Won’t be able to move their arms to touch their nose (C5-C6).
  6. Patient will have a weak cough or will be unable to speak.
  7. Respiratory signs include: Saturation < 90%, Respiratory Rate
    <12 /min, or apnoea. 
  8. Loss of consciousness.
